Certificate of Authenticity

I have been invited to be on exhibit in my first gallery showing!  I also have a few stores and gallery/frame shops that want to sell my work.  Can anyone direct me to a good resource for creating my own certificate of authenticity or a place that offers templates?  I remember Gavin talking about it on a PPS podcast but I don't remember how he came up with the COA.
